What is CDS?

Consumer Directed Services (CDS) is a Missouri Medicaid-funded program that empowers individuals with disabilities or chronic health conditions to live independently in their own homes. Instead of relying on a traditional home health aide sent by an agency, CDS lets you choose your caregiver — often a family member, friend, or someone you trust.

As a CDS participant, you’re in charge. You direct your care. We take care of the paperwork.

Who Is Eligible?

To qualify for CDS in Missouri, you must:

  • Be 18 years or older

  • Be eligible for MO HealthNet (Missouri Medicaid)

  • Be physically disabled and require assistance with daily tasks

  • Be capable of directing your own care or have a designated representative

  • Live in a non-institutional setting (not in a nursing home)

Who Can Be My Caregiver?

You can choose almost anyone you trust, including:

  • Adult children

  • Relatives

  • Friends

  • Neighbors

Note: Your spouse cannot be your paid caregiver, per Missouri rules.

  • Meal preparation

  • Mobility & transfers

We serve as your Fiscal Intermediary — which means we handle:

  • Background checks for caregivers (FCSR, E-Verify)

  • Payroll and taxes

  • Timesheet tracking and Electronic Visit Verification (EVV)

  • Ongoing support and training

  • Help with care planning and state compliance
